Introduction


This tool presents the proportion of all malignant cancers (excluding non-melanoma skin cancer)* which present as an emergency in England. The proportions are provided, by financial quarter, for Clinical Commissioning Groups (CCG), Cancer Alliances (CA) and a range of cancer sites.

Pages CCG variation, Cancer Alliance variation and Cancer site variation contain a figure and a table containing the quarterly proportion of emergency presentations and a 1-year rolling average.

The Emergency Presentation metric shows the estimated proportion of cancers which present as an emergency. This is an important driver of cancer outcomes: patients with cancers that present as an emergency suffer significantly worse outcomes. The recent cancer strategy for England recommended that the proportion of emergency presentations should be regularly reported and reviewed. The metric estimates the true proportion of emergency presentations using first admissions to hospital as a proxy for diagnosis to allow more rapid reporting.

* In situ breast cancer is provided in the data download but is not included in this tool due to consistently low number of emergency presentations.

Datasource

Data presented is taken from the National Cancer Registration Dataset ( National Cancer Registration Data Profile ) Cancer Analysis System, snapshot 2109 .

Populations data has been sourced from ONS Mid 2018 Lower Super Output Area Population Estimates, released 09/09/2020.

Postcode lookups are as defined in the ONS National Statistics Postcode Lookup (NSPL) as of April 2021.

Geography names and codes are as defined on ONS Open Geography Portal as of April 2021.

The chart shows the proportion of all malignant cancer diagnoses (excluding non-melanoma skin cancer) which were emergency admissions for the selected CCGs and reference area. Confidence intervals are provided for quarterly proportion and 1-year rolling average.

Table 1 provides the data for the chosen CCG. Table 2 provides populations for the chosen CCG and area chosen for comparison.

Select a main CCG of interest and a reference area to compare against. These must be different areas.

Cancer Alliance

Cancer site

The chart below shows the proportion of first hospital admissions (and confidence intervals) which were emergency admissions for different cancer sites for the whole of England. Please note that "Breast in situ" is not shown due to consistently low numbers of emergency presentations.

Table 1 provides the data for the chosen cancer site.

Methodology

Data are drawn from the Hospital Episode Statistics Admitted Patient Care data and linked to cancer registration data.

Numerator

Number of first inpatient admissions having a diagnostic code indicating a presentation of cancer which has an emergency method of admission. Note that persons with a prior registration or prior admission of cancer of a similar type are removed from the numerator.

Denominator

Number of first inpatient admissions having a diagnostic code indicating a presentation of cancer. Note that persons with a prior registration or prior admission of cancer of a similar type are removed from the denominator.

Crude proportion

The number of first inpatient admissions, having a cancer diagnostic code indicating a presentation with an emergency method of admission, divided by, the number of first of inpatient admissions, having a cancer diagnostic code, multiplied by 100.

Confidence Intervals

Wilson score method is used to give 95% confidence intervals for all the proportions presented.
